But then I realized, though my circumstances may have changed, I haven't.
Dr. Lisa Cassileth (0:12)
And the things that matter to me haven't changed either.

I'm Jessica Cruel, Editor-in-Chief of Allure and host of the Perspectives Podcast. One of the reasons I was so excited to do the series was for this very episode. This time, we're talking all about plastic surgery, both cosmetic and medical. And honestly, plastics is one of my favorite things to report on at Allure. We always talk about trends and technological advancements, what's happening right now, and of course, what comes next.
Dr. Lisa Cassileth is a renowned plastic surgeon in Beverly Hills. She's known for her minimally invasive reconstructions, her ability to fix botched boob jobs, and of course, her hyper focus on patient outcomes. If you ever thought, I think I'm going to get a boob job. What should I ask for? What details should I pay attention to? What are the red flags I should watch out for? This is the conversation for you.
And if you've never had that question, well, I still think this is the conversation for you, because we get into so much of the science behind breasts. So listen in. This is Perspectives.
Dr. Cassileth, thank you so much for being here.
Dr. Lisa Cassileth (1:38)
It's my pleasure. Thanks for having me, Jessica.
Jessica Cruel (1:40)
So, of course, the first thing I want to know is what are the women asking for? What's popular? What's the most common request you're getting right now?
Dr. Lisa Cassileth (1:49)
I would say for me myself, I'm known as more of an innovator for breast reconstruction. I do cases that need to be now redone, that were done years ago. So what I'm seeing is a lot of patients with like, for example, that might have had a breast implant placed 20 years ago where we're taking that out and redoing it in a new way.
I see a lot of patients that are coming in with weight loss, and that might have been a reduction patient 10 years ago. Now I'm seeing a lot of people like, okay, my body is great. What the heck is happening with my boobs? So hanging down to the bell line is not very fun. And then I'm seeing a lot of women requesting reconstruction where they could have a simple, safe surgery. And there's a lot of delving of the patient into AI and into the internet prior to our consultation. So they know what to ask, and it's making it a better experience for them.
Jessica Cruel (2:41)
We know that in beauty and health and plastic surgery, one of the biggest things that's happening right now is the rise of GLP-1 usage, right? And when you mentioned people feeling great about their bodies and then all of a sudden they're like, but we got to do some refining. I think that's very interesting.
Are you linking that to GLP-1 usage?
Dr. Lisa Cassileth (3:09)
Absolutely. I mean, in Los Angeles, which is the hub of body conscious community, I think in the United States, everyone's got personal trainers, they're on their own agenda. They're now combining that with peptides, ozempic or more commonly as zep-bound or for zephytide-like peptide, and a new one red-off, which just is coming online from pharmacies. And so many of these patients are coming in a wait they haven't been since college.
So the consequences of what happened in the interim are weighing on them different ways. The most common thing I see is women aren't really getting breast reductions as much as they used to. They're getting breast lifts because after the weight is lost, now the volume works for their smaller body, but it's in the wrong place. You'll have no breasts on the upper half and all the breast is on the lower half. It's really now a skin problem and the loss of like the gland used to be holding all this fat.
Now, it's just sagging down to the bottom. It's like a sad reminder of our old breasts. I didn't used to see that many breast lifts and reductions in an older population, 40s, 50s, 60s. People who have bad extra breast size their whole lives are now coming in after weight loss, not so much for a reduction, but for a lift because now they have so much excessive skin. They're losing traditionally women that are not even that overweight, they're losing 20, 30 pounds and it's hitting them in the breast. So it's really creating an aesthetic gap from where they want to be.
